Health Information
This website has no patient accounts and no document uploads, and it does not transmit health information. The tremor questionnaire on the tremor page runs entirely in your browser: your answers are turned into a summary you print or save yourself, and nothing is sent to this website or to anyone else. The downloadable questionnaire PDF is filled in on your own device. Do not send sensitive health information through ordinary email. Appointment requests and medical records should be handled through the medical center’s established secure channels.
Preparation Checklist
The checkboxes on the appointment page are a temporary preparation aid. The website does not send or save those selections, and they do not determine treatment eligibility.
Links And Services
Links to PubMed, publishers, Google Maps, and other services open content governed by those services’ policies. Fonts are delivered through Google Fonts, which receives the information normally associated with a web request. Embedded maps are provided by Google Maps and may process connection information when loaded. The website host may process connection and access information needed to operate and protect the site.
